Photo & Video Release

I hereby grant Friendship Circle of Virginia permission to photograph and/or record me and/or my child/ward during programs, events, or activities hosted by Friendship Circle of Virginia.

I understand that these photographs and recordings may be used for promotional purposes including but not limited to use on the FCVA website, FCVA social media, and FCVA printed materials.

I understand that no compensation will be provided for the use of these images or recordings, and I release Friendship Circle of Virginia from any claims related to the use of such photographs or recordings.

I certify that I am the parent or legal guardian of the minor named below (if applicable) and that I am authorized to give this consent on their behalf.
